(A) You must submit a complete IRB Application. This must include a completed IRB Application Form (Limited Review). A signed approval by your CJ450 instructor, An Informed Consent Form to your respondents; and, copy of the survey that you will utilize.

This must be completed by the end of Unit 3. Once completed, your instructor will provide you with the listing of names for your respondents. Failure to complete this on time will result in zero points for this assignment.

ALL forms MUST be in MS Word, so I can edit them.

Consent form

Supervisor form

Your Survey

And then emailed to my Park email as well.

(B) Post a draft outline (table of contents) for your thesis including possible appendices, tentative charts or tables, chapters, etc.
