Vignette Analysis II:
This assignment focuses on vignette analysis and direct application of course concepts to the persons and situations presented in the vignette for each question. All discussions must take into account the legal and ethical considerations, as well as issues of culture and human diversity that may pertain to the situations presented below. You must include how your own cultural beliefs, values, possible biases and stereotyping may affect your clinical perspective. Culture and Legal/Ethical issues can be found in the course texts, but you are encouraged to use outside sources to enhance your work by providing supportive documentation.
Please keep your responses focused on what is presented in the vignette. Do not add information but use your creativity to support what you see in the vignette as written.
All assignments MUST be typed, double-spaced, in APA style and must be written at graduate level English. The content, conciseness and clarity of your answers will be considered in the evaluation of your work. The average length of answers to each question is 2 – 3 pages, although some answers may vary. You must integrate the material presented in the text and cite your work according to APA format. Use of the internet and the University Virtual Library is encouraged.
Andrea, a 30 year old woman of Asian and Middle Eastern descent. She is referred to you by a colleague for individual treatment. This colleague, who is working with the family, would like you to evaluate Andrea for in-patient treatment. She is experiencing auditory and visual hallucinations and persecutory delusions. She has a history of polysubstance dependence, including intravenous drugs. In her youth, she was arrested several times for shoplifting. Andrea is currently violent and paranoid. It is not known whether this is related to substance abuse or an active phase of schizophrenia or some other psychotic disorder.
1. Considering a differential diagnosis, what is your initial assessment and diagnostic impressions of Andrea and why? How might sociocultural factors affect your assessment? Consider the psychotic spectrum disorders and bi-polar disorders concurrent with substance related disorders in your response. Please present other ideas that you have for Andrea.
2. For Andrea, consider the Six Dimensions Checklist in Appendix E in “The Addiction Treatment Planner” and the Mental Health Screening Form III and Simple Screening Instrument for Substance Abuse in Appendix H of “Substance Abuse Treatment For Persons With Co-Occurring Disorders” * see note below
Using the vignette information, present a summary of the completed checklists and screening forms for Andrea in consideration of your diagnostic impression. Integrate both treatment and pharmacological interventions. Remember to include culture in your treatment planning as well as legal/ethical issues that might be present.
* Note: You do not have to present the actual checklists or assessment forms. Just write a summary of the responses so that I can review them. Appendix H is located in TIPS 42. You may use the DSM-5 for diagnosis references
